• Care Management calls include assisting calling doctors, pharmacies, calling health insurances, struggling with decisions, etc.
• No Initial assessment needed.

More calls are available depending the needs of your loved one.

INITIAL ASSESSMENT NEEDED 

This Bridge Plan emphasizes in supporting your loved at home, ensuring all needs are met: caregiving agency selection, family support, behaviors. While families continue with their commitments, a Bridge Care Manager will ensure your loved one needs are met.

INITIAL ASSESSMENT NEEDED

This plan is for adults who need guidance with coordinating  some areas of their lives.  This is our Geriatric Care Management Program.

Family members can choose this plan who live far away from their loved ones or have commitments that do not allow them to spend enough time with them. The older adult  will have full on-going support with the assistance of a Bridge Manager.
