We coordinate all services for adults and their families, providing piece of mind that your loved one will be supported through this new journey.
SERVICES
This Bridge Plan emphasizes in supporting your loved at home, ensuring all needs are met: caregiving agency selection, family support, behaviors. While families continue with their commitments, a Bridge Care Manager will ensure your loved one needs are met.
Through Person Centered Care, a Bridge Care Manager will assist with the following:
AGING IN PLACE - Coordination of all services to remain at home.
- Personalized care plan while building a strong circle of support.
- In person visits to ensure all needs are met.
- Choosing the right caregiving agency. Caregiver Education and Training (behaviors). The Bridge Care Manager will train and supervise agency caregivers' responsibilities. The Bridge Care Manager will assist to choose the right Caregiving Agency.
- Caregiver education and training to understand resistive behaviors and unpredictable behaviors.
- Sundowning - How to be prepared when dealing with emotions, behaviors and triggers.
- Effective communication - how to talk to our loved ones so they remain calm while minimizing anxiety. How to avoid unnecessary confrontations.
- Educating and supporting family members with the disease progression.
- Technology Assessment - wearable and non-wearables based on the needs of the adult.
- Home Safety Assessment.
- Incorporating purposeful activities. Music, art, and pet therapy.
- Assisting with locating the right memory care facility if needed.
HEALTH CARE MANAGEMENT
- Explaining Health Insurance benefits. Understanding Health Insurance changes, co-pays, letters, and navigating the complex Medicare System.
- Health care plans: assisting with Health Insurance calls/information.
- Assisting with grievances and appeals with Health Insurance Companies.
- Scheduling medical appointments, escorting older adults to medical appointments, and ensuring doctors' directions are followed.
- Ensuring medications and new medications are refilled on time. Reminders for medication administration/calling pharmacies.
- Assisting with hospitalizations and safe discharge plans.
ADVOCACY
- Scams - Elder abuse prevention. Fraud prevention. Identifying fraud mail, calls.
- Explanation and guidance in the completion of Dementia Directive, Power of Attorney for Health Care, Wills, Advanced Health Care Directives, Post, 5 Wishes, Polst.
WELLNESS PROGRAMS
- Assisting with nutrition and exercise programs.
- Meal reminders - Grocery shopping coordination.
- Guidance to achieve health care goals.
SOCIALIZATION
- Monthly Social Online Cafe gatherings
- Monthly online activities
