Coordinated Family Support Services “CFS”

WHILE REMAINING IN THEIR FAMILY HOME

CFS is a specialized service designed to support adults 18 and older, as well as parents and aging caregivers, by addressing their evolving care and coordination needs.
- Adults choose to live in their family home
- Adults are served by a Regional Center
- Assistance to Parents and Aging Caregivers to access to services in the community.
For Aging Parents: Assistance to aging parents experiencing cognitive impairment or dementia-related symptoms to access community-based supports, healthcare services, and coordinated resources that promote safety, stability, and overall well-being.
CFS assists with the coordination of services and supports that allow adults to continue living in their family home such as:
- Coordinating & getting services delivered, including help with generic resources.
- Medi-Cal applications, IHSS applications, CalFresh and SSI applications.
- Assisting adults access their community and become more independent.
- Assistance with College applications, navigate their preferred college and Student Disabilities Department requirements.
- Assistance with developing communication skills with their families and out in the community. Learn to set boundaries, express needs, and build confidence.
CFS providers will prepare a service plan to help reduce or remove barriers to staying in the family home, address challenges, and meet goals.
Services will be provided in a manner that respects the language & culture of individuals and their families, and services will be tailored, individualized, and flexible to meet individuals’ changing needs and preferences.
