This is our Aging Life/Geriatric Care Management Program
that will cover all older adults’ needs based on Person-Centered Care Approach.

Health Care Advocacy
Our care managers accompany clients to medical appointments and advocate for essential treatments, procedures, and specialist visits to ensure comprehensive care. Social Bridges 4 Life does not transport clients. Care Managers meet clients at Doctor’s offices.

Liaison Support
When families live at a distance, care managers act as the central contact among healthcare providers, keeping everyone updated on changes and developments in the client’s condition.

Financial Guidance
We assist with understanding complex insurance matters and can help clients manage and organize bill payments. All bill payments will be previously discussed with family members.

Housing Transitions
When a higher level of care is needed, care managers guide families in selecting appropriate housing options that meet the client’s evolving needs.

Access to Resources
Our professionals maintain an extensive network of trusted service providers, including elder law attorneys, financial planners, mobile notary services, and other services needed.

At Social Bridges 4 Life, all care managers hold at least a degree in social work or a related human services field, ensuring clients receive knowledgeable and compassionate support.
