This is our Aging Life/Geriatric Care Management Program
that will cover all older adults’ needs based on Person-Centered Care Approach.

A Bridge Care Manager will assist with:
*Guiding and making choices based on your preferences and needs. We assist with Healthcare coordination, overall coordination of services in all areas needed, education, monitoring caregiving agencies (if needed).
*Assisting in navigating the changes of your life course and maximizing your quality of life.

This Plan is for adults who need guidance in coordinating
some areas of their lives.

Family members can choose this plan who live far away from their loved ones or have commitments that do not allow them to spend enough time with them. The older adult will have full ongoing support with the assistance of a Bridge Manager.
